What you will be doing
- Build real professional relationships with the young people you support, enabling them to enjoy activities both at home and out in the community
- Contribute to care plans, risk assessments, and reviews, then actually implement the plans that help each person move forward
- Meet their emotional and physical needs day to day while maximising opportunities for their development and independence
- Help them develop life skills and encourage independence in practical ways, from keeping the home clean and tidy to participating in daily activities
- Work with families and external agencies to give each young person the joined-up support they need
- Keep detailed, high-quality records and maintain the standards of professionalism the service and its stakeholders expect
